Medicaid

• You may need a letter of necessity from your child’s medical provider

Disability Parking Placard

• These can be helpful for students who may elope and/or have safety concerns especially when in a busy parking lot

• Talk to your child’s primary care provider about getting a Parking Placard. You will need the provider to fill out the medical certification portion of the form.
